At Summit, we offer convenient, easy ways to report injuries 24 hours a day, 7 days a week—by phone, email, fax, or online through our website. Our own staff takes your report, so we get involved with claims immediately and are here to help you through the process.

Easy online reporting

Have you seen our online injury reporting tool? We heard from policyholders that you wanted to report an injury with less information required. So, we launched our revamped tool, which only requires:

  • Policy number
  • Name and contact phone number of the person reporting the injury
  • Injured employee’s first and last name and their work location
  • Injury date

Of course, the more information we have, the better we are able to manage the claim and facilitate medical treatment of your injured employee. Still, it’s better to report an injury as soon as possible and provide any missing information later. And our own employees receive the reports, so we get involved with claims immediately and are here to help you and your injured employees through the process.

Important OSHA notification requirements

All employers are required to notify OSHA in the event of an on-the-job death or if an employee suffers a work-related hospitalization, amputation or eye loss.

How to notify OSHA:

  • Call OSHA’s toll-free and confidential number at 1-800-321 OSHA (6742).
  • In person by visiting the nearest OSHA office.

Notification deadline information, per OSHA’s Recordkeeping Regulation, 29 C.F.R. § 1904.39 (2018):

  • An on-the-job death must be reported within eight hours
  • An on-the-job amputation, eye loss or in-patient hospitalization must be reported within 24 hours
