The following toll-free telephone number is available to all Heritage contracted providers: 1-800-282-7644. Providers may call this number to access workers’ compensation payor summary information and a current list of lessees of the network.

This web page provides you with notice that the Heritage provider network may be leased to other payors or agents, including workers’ compensation insurers or self insured entities. These payors or agents may be permitted to pay a fee less than the workers’ compensation fee schedule.

Of course, if an additional payor is granted access to the Heritage network, you will be notified. Regardless, this web page will be updated at least twice a year to include any additions or deletions. In the event that working with an additional payor causes an issue for you or your practice, you maintain the right to opt out of the network.
