Credentialing

How does a medical provider become credentialed or recredentialed with Summit?
To complete the initial credentialing process, a provider must submit the following information to a Heritage representative.

  • Completed application (click here to get a copy of the credentialing application)
  • Copies of the physician’s DEA license and medical license
  • Proof of medical malpractice insurance
  • State workers’ compensation certificate
  • Copies of board certification (if board-certified)
  • Copies of medical diploma, internship and residency (if not board-certified)
  • Curriculum vitae
  • Current W-9

Every three years, providers must be recredentialed. At this point, providers must submit the following documents.

  • A new, completed application (click here to get a copy of the recredentialing application)
  • Copies of DEA license and medical license
  • Proof of medical malpractice insurance
  • A new W-9
