Report an Injury
Workplace injuries can happen anytime, day or night. That’s why Summit gives you three ways to report injuries 24 hours a day, 7 days a week—by phone line, fax line or online.
Important reminder for Florida, Kentucky and Louisiana employers
Be sure to give injured employees a workers’ compensation identification card to present to the treating medical facility. Check your Claims Kit or print a card now. If you need more cards, please contact the Claims Center in your region.
